Most numerology numbers require some arithmetic — summing letters, reducing a full birth date. The birthday number skips most of that. It's simply the day of the month you were born, full stop, before any reduction happens.
From there, some numerologists reduce a two-digit day (like 23) down to a single digit (2+3=5), while others leave two-digit birthday numbers as they are, treating 10 through 31 as their own set of meanings. Both approaches exist side by side.
